I encountered this AMD problem report web form today which has “Title” as a required field, with the only options being “Company”, “Mr” or “Ms”. First of all, why does AMD need to know my gender? Are they going to triage my technical problem differently based on whether I’m a male or female? Second, my title is not on that list. I realize that’s pedantic, but why call the field “Title” if they really just mean “Gender”? So this ill-designed form ticked me off for no useful purpose at all.

Ha! It gets better. The automated response email just says “Dear Customer,”
